Extension/Amendment-H-1B Client Requirements:

- Documentation verifying the Federal Employer Identification Number (FEIN) of the organization.
- A copy of the Certificate of Incorporation.
- Copies of the latest federal tax return for the company.
- If the beneficiary will be working off-site, include the beneficiary's employment contract with the company, a purchase order or contract, a vendor agreement between the petitioner and the client site, along with an itinerary.
- Total number of current employees, including those holding H-1B or L-1 status.
- Details regarding the beneficiary's proposed job title, annual salary, and job description.
- Name, title, contact information, and email address of the individual signing the petition.
- Locations where the beneficiary will be employed, as well as the address and phone number of the company's headquarters.

Foreign National Documentation Requirements:

- Current residential address in the U.S.
- Residential address in the country of origin
- Social Security Number (SSN)
- Copies of the three most recent pay stubs (the last pay stub must be from the latest pay period, whether biweekly, semimonthly, or monthly)
- Copy of the biographic (photo) page of the passport
- Copy of the front and back of the latest I-94 card
- Copy of all U.S. visa stamps present in the passport
- Copy of the entry stamp in the passport from the initial entry to the U.S. under H-1B or L-1 status
- Copies of all I-797 approval notices for H-1B, L-1, or any other visa categories previously issued
- Copies of all prior U.S. visa stamps, I-797 approval notices, and any other relevant documentation pertaining to all nonimmigrant statuses previously held in the U.S.

For individuals who attended school in the United States, please provide copies of F-1 visa stamps, all I-20 forms associated with F-1 status, and the EAD card related to OPT, if applicable.

For those who were in J-1 status, include copies of J-1 visa stamps and all DS-2019 forms (or IAP-66 forms) issued for that status, along with the I-601 waiver for the two-year residence requirement, if applicable.

Additionally, submit copies of all U.S. visa stamps for any other nonimmigrant visa statuses, such as B1/B2, H-4, L-1, L-2, etc.

Please provide a copy of your diploma and transcripts/marksheets for each post-secondary degree, as well as a copy of the educational credentials evaluation from previous petitions, if available.

For those lacking four full years of university education, work experience letters must demonstrate three years of relevant work experience for each missing year of study. These letters should be on company letterhead, signed by an authorized company representative, and must include your job title, employment dates, weekly hours (or indicate full-time status), along with a description of job responsibilities.

Dependent Requirements:

For a dependent spouse without an independent visa:

- A copy of the marriage certificate registered with civil authorities, along with a certified translation if available.
- A copy of the biographic (photo) page of the passport.
- A copy of the front and back of the most recent I-94 card.
- Copies of all U.S. visa stamps in the passport.
- A copy of the entry stamp for the initial entry into the U.S. under H-4 or L-2 status.
- Copies of all I-797 H-4 approval notices ever issued.
- Copies of all previous U.S. visa stamps, I-797 approval notices, and any other documentation related to all nonimmigrant statuses previously held in the U.S.

For Dependent Children:

Copy of Birth Certificate registered with the civil authorities, with certified translation (if available)
Copy of Biographic (photo) page of the passport
Copy of front and back of most recent I-94 car
Copy of all U.S. visa stamps in the passport
Copy of entry stamp in the passport for the first entry to the U.S. in H-4 or L-2 status
Copy of all I-797 H-4 approval notices ever issued
Copies of all previous U.S. visa stamps, I-797 approval notices, and other documentation related to all nonimmigrant status previously held in the U.S.
