Company Requirements for H-1B Amendment/Extension:

- Documentation verifying the Federal Employer Identification Number (FEIN) of the organization
- A copy of the Certificate of Incorporation
- Copies of the latest federal tax return for the company
- If the beneficiary will be working off-site, include the beneficiary's employment contract with the company, a purchase order or contract, a vendor agreement between the petitioner and the client site, along with an itinerary
- Total number of current employees, including those holding H-1B or L-1 status
- Proposed job title, annual salary, and job description for the beneficiary
- Name, title, contact information, and email address of the individual signing the petition
- Locations where the beneficiary will be employed, as well as the address and phone number of the company's headquarters.

Requirements for Foreign Nationals (H-1B Extension/Amendment)

- Current residential address in the U.S.
- Permanent address in the home country
- Social Security Number (SSN)
- Copies of the three latest pay stubs (the final pay stub should be from the most recent pay period, whether biweekly, semimonthly, or monthly)
- Copy of the biographic (photo) page of the passport
- Copy of the front and back of the most recent I-94 card
- Copy of all U.S. visa stamps present in the passport
- Copy of the entry stamp for the initial entry into the U.S. under H-1B or L-1 status
- Copies of all I-797 approval notices for H-1B or L-1 status ever issued
- Copies of all previous visa stamps, I-797 approval letters, and supporting documentation for every nonimmigrant status held in the U.S.
- Copies of F-1 or M-1 visa stamps, all I-20 forms issued related to F-1 or M-1 status, and the EAD card issued for OPT (if applicable) for students who studied in the U.S.
- For individuals with J-1 status, copies of all J-1 visa stamps, all DS-2019 forms (or IAP-66s) related to that status, and, if applicable, an I-601 waiver for the two-year residency requirement.

- Copies of all U.S. visa stamps (B1/B2, H-4, L-1, L-2, etc.) for other nonimmigrant visa statuses.

- Copies of diplomas, transcripts, and marksheets for all post-secondary degrees, along with any educational credential evaluations from previous petitions (if available).
- Letters of work experience demonstrating three years of professional experience for each missing year of university education (these letters must be on company letterhead, signed by an authorized company representative, and include job title, employment dates, weekly hours worked or full-time status, and a description of job responsibilities) for those lacking four full years of university education.

Dependent Requirements (H-1B Extension / Amendment)

For Dependent Spouse (without an independent visa):

- A copy of the Marriage Certificate registered with civil authorities, along with a certified translation if available.
- A copy of the Biographic (photo) page of the passport.
- A copy of the front and back of the most recent I-94 card.
- A copy of each U.S. visa stamp present in the passport.
- A copy of the entry stamp in the passport from the initial entry to the U.S. under H-4 or L-2 status.
- A copy of every I-797 H-4 approval notice that has been issued.
- Copies of all previous U.S. visa stamps, I-797 approval notices, and any other documentation related to all nonimmigrant statuses previously held in the U.S.

For Dependent Child(ren) of the Spouse:

- A copy of the Birth Certificate registered with civil authorities, with a certified translation if available.
- A copy of the Biographic (photo) page of the passport.
- A copy of the front and back of the most recent I-94 card.
- A copy of all U.S. visa stamps in the passport.
- A copy of the entry stamp in the passport from the initial entry to the U.S. under H-4 or L-2 status.
- A copy of all I-797 H-4 approval notices that have been issued.
- Copies of all previous U.S. visa stamps, I-797 approval notices, and any other documentation related to all nonimmigrant statuses previously held in the U.S.
